Remote monitoring treatment management services are provided when a physician or other qualified medical professional uses remote monitoring results to treat a patient as part of a specific treatment plan. The service must be ordered by a physician or other qualified medical professional.

CODE DESCRIPTION

CPT 98975 is reported for patient education regarding devices used for initial device setup and data collection. Code 98975 can only be billed once per therapy episode. A therapy episode begins when the remote monitoring service is activated and ends when a specific therapeutic goal is achieved.

CODING CRITERIA

  • CPT 98975 is used to report for device initial setup, patient education, and usage.
  • CPT Code 98975 is subject to de minimis standards when delivered by a therapist assistant.
  • Data Device availability for scheduled recording or sending alarms is reported every 30 days.

BILLING CRITERIA

  • All RTM services should be provided as part of the treatment plan.
  • CPT Code 98975 is billed once per care episode.
  • CPT Code 98975 will not be charged if the patient owns the device used for monitoring.
