BILLING CRITERIA
CPT 99458 is billed for every 20 minutes spent with a patient after reporting CPT 99457, up to a total of 80 minutes per month.
REIMBURSEMENT CRITERIA
CPT code 99458 permits for a reimbursement of $40.84.
